  • All intake appointments are currently virtual.

  • Click on the “Request Appointment” button below and follow the prompts to request an appointment that fits your schedule.

  • If the appointment time you desire has a phone icon next to it - try going to the next week to find an available time.

  • Within one business day, you will receive intake paperwork, which will take 10-20 minutes to complete.

  • After CFS receives the intake paperwork and verifies insurance benefits, appointments will be approved. We request that intake paperwork be completed within 24 hours of you receiving the paperwork for us to continue to hold that time slot for you. If we need to change your appointment due to incomplete paperwork or any other reason - we will notify you by email or by calling.

  • Please check your email and spam for an email with intake documents. If you have not received the intake paperwork after one business day of your request - please email hello@connectedfamilyservices.com for support.
